Dear Parent/Guardian,

The end of the school year is quickly approaching. All medications kept in the school clinic must be picked up by 12:00 pm onJune 1.  All medications left past this date will be discarded.

Please sign the permission slip and send it back to me if you would like for your student to pick up his/her over-the-counter medication or inhaler on the last day of school. All other PRESCRIPTION medication MUST  be picked up by a parent or guardian.

Remember, new medication forms must be filled out and signed by a parent or guardian and physician each new school year.

  • An Asthma Action Plan is required for students with asthma.
  • Severe Allergy Action Plan is required for students with EpiPens.
  • Medication Administration Form is required for ALL prescription and ALL over-the-counter medications.
  • Special Procedures Form may be required for other special needs.
  • Diabetic Management Plan from your student’s physician is required for all students with diabetes.

o   Please make an appointment to meet with the school nurse prior to the start of school if your child has diabetes.

o   BISD Diabetic form is not required for Cook Children’s Endocrinology patients.
